Region Gävleborg launches self-monitoring via “Min vård Gävleborg”

Region Gävleborg is now strengthening its digital healthcare offering by introducing self-monitoring. The initiative is being carried out in collaboration with Platform24 Healthcare AB. The new service will become an integrated part of the region’s existing digital gateway, Min vård Gävleborg, making it easier for patients to be actively involved in their care.

Platform24’s technology is currently used as the platform behind Min vård Gävleborg. By adding self-monitoring to the same app already used for digital visits, the foundation is laid for a more unified experience for both residents and healthcare professionals.

“Region Gävleborg already has a strong digital gateway for its residents through Min vård Gävleborg. By integrating self-monitoring into a platform that is already widely used, healthcare providers can work more proactively and patients can gain greater peace of mind in their everyday lives, from the first contact through to follow-up,” says Tobias Perdahl, Co-CEO and Specialist Physician at Platform24.

More person-centered care The rollout initially focuses on patients with heart failure. Through self-monitoring, patients can register metrics (such as blood pressure and weight) and respond to self-assessments directly on their mobile phone or tablet. The data reaches the healthcare providers in real time, meaning medical interventions can be initiated more quickly at the first sign of deterioration. This can reduce the need for physical hospital visits and create a sense of security in everyday life.

“We want the patient to feel involved and supported in their treatment, even between healthcare visits. When we follow their progress together, it can contribute to both increased security and a more person-centered care experience,” says Anders Lundberg, Physician and Operations Manager for the Cardiology Department, Region Gävleborg.

During the spring, self-monitoring will also be launched for patients with bipolar disorder as well as for patients with prostate cancer.
